Results obtained statistically indicate a fairly high frequency of hand injuries, including open ones. Often, an incorrect prescription of antibiotics in the form of unjustified antibiotic therapy in the absence of direct indications is done.</p> <p><bold>Aim. </bold>This study aimed to develop a rational algorithm for perioperative antibiotic prophylaxis for open hand injuries in children based on experimental and clinical research.</p> <p><bold>Materials and methods. </bold>The study was divided into two phases: experimental and clinical. The experiment was carried out on 60 sexually mature, outbred guinea pigs. During the experiment, the analysis of clinical signs of inflammation, complete blood count and biochemical blood analysis, spiral computed tomography, and morphological study of the consolidation zone were performed. The clinical part of the work is based on the analysis of surgical treatment results of 120 children with open hand injuries, which were divided into two groups: the main group that included 50 children receiving treatment according to the experimentally developed perioperative antibiotic prophylaxis (PAP) algorithm, and the comparison group included 70 children receiving a perioperative course of antibiotics from 0 to 7 days, without a well-defined algorithm.</p> <p><bold>Results.</bold> In the analysis of clinical results, laboratory, radiation, and morphological methods of studying the experimental material, the absence of antibiotic prophylaxis in open fractures of small tubular bones with a 90% probability was found to entail pro-inflammatory complications in the area of postoperative wound; however, a one day short-term course of PAP will prevent complications and will not have statistically significant differences with a longer course of three days.</p> <p><bold>Discussion.</bold> Results of a clinical study proved the effectiveness of the developed algorithm and showed better results in early and late postoperative period in comparison with the chaotic prescription of antibacterial drugs without a clear algorithm; however, no statistically significant differences in the level of postoperative complications were found.</p> <p><bold>Conclusion. </bold>On the basis of an experimental model of an open fracture of the tubular bone, osteosynthesis and various courses of antibiotic prophylaxis has proven that a short-term course of PAP within 24 h is the most optimal one, and achieving minimal risks of inflammatory complications and better anatomical and functional outcomes in clinical practice.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>На сегодняшний день повреждение кисти — одна из наиболее актуальных и часто встречающихся проблем современной травматологии во всем мире. Получаемые результаты статистически свидетельствуют о достаточно высокой частоте повреждений кисти, в том числе открытых. Зачастую имеет место некорректное назначение антибиотиков в виде необоснованной антибиотикотерапии при отсутствии прямых показаний.</p> <p><bold>Цель. </bold>Разработка рационального алгоритма периоперационной антибиотикопрофилактики при открытых травмах кисти у детей на основе экспериментального и клинического исследования.</p> <p><bold>Методы. </bold>Исследование было разделено на два этапа: экспериментальный и клинический. Эксперимент проводили на 60 половозрелых, беспородных морских свинках. В процессе эксперимента проводили исследования клинических признаков воспаления, общего и биохимического анализа крови, спиральной компьютерной томографии и морфологическое исследование зоны консолидации. Клиническая часть работы основана на анализе результатов хирургического лечения 120 детей с открытыми травмами кисти, которые были поделены на две группы: в основную группу вошли 50 детей, получавших лечение по экспериментально разработанному алгоритму периоперационной антибиотикопрофилактики, а в группу сравнения вошли 70 детей, получавших периоперационный курс антибиотиков от 0 до 7 дней, без четко выработанного алгоритма.</p> <p><bold>Результаты. </bold>При анализе результатов клинических, лабораторных, лучевых и морфологических методов исследования экспериментального материала было установлено, что отсутствие антибиотикопрофилактики при открытых переломах мелких трубчатых костей с вероятность 90 % повлечет за собой гнойно-воспалительные осложнения в области послеоперационной раны, однако краткосрочный курс периоперационной антибиотикопрофилактики в течение 1 сут позволит предотвратить осложнения и не имеет статистически значимых различий с более длительным курсом в 3 сут.</p> <p><bold>Обсуждение</bold>. Результаты клинического исследования доказали эффективность разработанного алгоритма и показали лучшие результаты раннего и позднего послеоперационного периода в сравнении с хаотичным назначением антибактериальных препаратов без четкого алгоритма, однако статистически значимых различий по уровню послеоперационных осложнений выявлено не было.</p> <p><bold>Заключение. </bold>На основании экспериментальной модели открытого перелома трубчатой кости, его остеосинтеза и различных курсов антибиотикопрофилактики доказано, что краткосрочный курс периоперационной антибиотикопрофилактики в течение 24 ч является наиболее оптимальным и в клинической практике позволяет добиться минимальных рисков воспалительных осложнений и лучших анатомо-функциональных исходов.</p></trans-abstract><kwd-group xml:lang="en"><kwd>antibiotic prophylaxis</kwd><kwd>open fracture</kwd><kwd>hand injury</kwd><kwd>children</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>антибиотикопрофилактика</kwd><kwd>открытый перелом</kwd><kwd>травма кисти</kwd><kwd>дети</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Kwak D, Lee S, Yoo J, et al.
