This review summarizes and analyzes data from international and national studies on the use of extracorporeal membrane oxygenation (ECMO) in newborns with CDH. We analyzed 153 publications from the PubMed, Cochrane, Springer, Clinical Key, and RSCI databases published between 2014 and 2025. Search keywords included ECMO, neonates, congenital diaphragmatic hernia, pulmonary hypertension, and mechanical ventilation. Only meta-analyses and randomized trials were included. Fifty articles on postnatal intensive care and ECMO use in infants with CDH were reviewed. Based on recent publications, this review provides an overview of ECMO techniques in newborns with CDH, including indications and contraindications, initial parameters, complications, methods for assessing efficacy, and weaning from ECMO. ECMO is a life-saving strategy that should be considered in children with CDH who do not respond to standard therapies; however, given the high risk of adverse effects and complications, the decision to use ECMO must be made on a case-by-case basis, guided by a personalized approach.</p></abstract><trans-abstract xml:lang="ru"><p>Врождённая диафрагмальная грыжа — порок развития, обусловленный наличием дефекта в диафрагме и перемещением органов брюшной полости в грудную клетку, требующий экстренной хирургической коррекции в неонатальном периоде. В обзоре обобщены и проанализированы данные международной и отечественной литературы об особенностях использования экстракорпоральной мембранной оксигенации у новорождённых с врождённой диафрагмальной грыжей. Проведён анализ 153 публикаций, входящих в базы данных Pubmed, Cochrane, Springer, Clinical Key, РИНЦ в период с 2014 по 2025 г. В процессе поиска использовали ключевые слова: экстракорпоральная мембранная оксигенация, новорождённые дети, врождённая диафрагмальная грыжа, лёгочная гипертензия, искусственная вентиляция лёгких. В поиск были включены только метаанализы и рандомизированные исследования. Для анализа использованы 50 статей, касающихся интенсивной терапии детей с врождённой диафрагмальной грыжей после рождения, возможностей применения использования экстракорпоральной мембранной оксигенации. На основании проведённого анализа публикаций последних лет представлена общая характеристика методов экстракорпоральной мембранной оксигенации у новорождённых с врождённой диафрагмальной грыжей, показания и противопоказания к её использованию, стартовые параметры, осложнения и методы оценки эффективности процедуры, методика отключения пациента от аппарата экстракорпоральной мембранной оксигенации. Экстракорпоральная мембранная оксигенация является жизнеспасающей терапевтической стратегией, которую целесообразно использовать у детей с врождённой диафрагмальной грыжей при отсутствии эффекта от стандартных методов лечения, однако, учитывая высокую вероятность нежелательных побочных эффектов и риск развития осложнений, решение об использовании данного метода терапии в каждом конкретном случае необходимо принимать, основываясь на персонифицированном подходе.</p></trans-abstract><trans-abstract xml:lang="zh"><p>先天性膈疝是一种发育缺陷，由膈肌存在缺损及腹腔脏器移位至胸腔所致，需在新生儿期进行急诊外科矫正。本综述汇总并分析了国际和国内文献中关于体外膜肺氧合在先天性膈疝新生儿中应用特点的数据。分析了2014年至2025年间纳入Pubmed、Cochrane、Springer、Clinical Key、RSCI数据库的153篇出版物。检索过程中使用的关键词包括：体外膜肺氧合、新生儿、先天性膈疝、肺高压、机械通气。检索仅纳入meta 分析和随机对照研究。分析使用了50篇涉及先天性膈疝出生后患儿进行重症监护时应用体外膜肺氧合可能性的文章。基于近年来发表的文献分析，呈现了先天性膈疝新生儿体外膜肺氧合方法的总体特征、使用适应证与禁忌证、起始参数、并发症及程序有效性评估方法、患者脱离体外膜肺氧合装置的方法学。体外膜肺氧合是一种挽救生命的治疗策略，当标准治疗方法无效时，可合理用于先天性膈疝患儿，但考虑到不良副作用的高发生率和并发症风险，在每个具体病例中是否使用该治疗方法需基于个体化方法决策。</p></trans-abstract><kwd-group xml:lang="en"><kwd>newborns</kwd><kwd>congenital diaphragmatic hernia</kwd><kwd>extracorporeal membrane oxygenation</kwd><kwd>pulmonary hypertension</kwd><kwd>pulmonary hypoplasia</kwd><kwd>review</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>новорождённые</kwd><kwd>врождённая диафрагмальная грыжа</kwd><kwd>экстракорпоральная мембранная оксигенация</kwd><kwd>лёгочная гипертензия</kwd><kwd>гипоплазия лёгких</kwd><kwd>обзор</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>新生儿</kwd><kwd>先天性膈疝</kwd><kwd>体外膜肺氧合</kwd><kwd>肺高压</kwd><kwd>肺发育不全</kwd><kwd>综述</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Russian Association of Pediatric Surgeons.
