derived from the three germ layers: endoderm, mesoderm, and ectoderm. The most common sites of teratomas are the sacrococcygeal region and ovaries (51%–60%). However, other locations are also possible: testes, the retroperitoneal space, the stomach, the neck, and the floor of the oral cavity. Teratomas in two different body cavities are extremely rare. A 15-year-old girl presented with a mass in the chest that was an incidental finding on routine chest X-ray. Subsequent inpatient workup (multislice CT and aortoarteriography) showed that the lesion had a thoracoabdominal location with a large feeding artery arising from the upper abdominal aorta. Surgery involved a right posterolateral thoracotomy through the 7th intercostal space. During the procedure, dense adhesion of the cystic mass to the right lower lobe was found. After mobilization of the tumor from the lung and ligation of its vascular pedicle, it was found that the tumor extended into the abdominal cavity through the esophageal hiatus. The diaphragmatic crura were divided, and the cyst was completely mobilized bluntly from the esophagus and stomach. The tumor was removed from the pleural cavity, and damaged tissues were repaired. Operating time was 155 minutes. There were no intraoperative complications such as bleeding or esophageal or gastric perforation. The chest tube was removed on day 2, and ultrasound showed no hydrothorax. Histopathology revealed a mature cystic teratoma. Follow-up multislice CT of the chest and abdomen showed no residual tumor. The relevance of this report lies in the extreme rarity of thoracoabdominal mature teratomas in children and the lack of standardized surgical guidelines. This description of successful radical resection of a large tumor via a single thoracotomy approach broadens the clinical understanding of the topographic anatomy of such tumors and demonstrates the feasibility of minimizing surgical trauma when the tumor involves two body cavities.</p></abstract><trans-abstract xml:lang="ru"><p>Тератомы — это новообразования, состоящие из клеточных элементов, происходящих из трёх зародышевых листков: энтодермы, мезодермы и эктодермы. Наиболее частая локализация тератом — крестцово-копчиковая область и яичники (51–60%). Однако возможны и другие локализации: яички, забрюшинное пространство, желудок, шея и дно ротовой полости. Тератомы в двух разных полостях тела встречаются крайне редко. Объёмное образование в грудной полости у девочки 15 лет было случайной диагностической находкой при проведении плановой флюорографии. В дальнейшем при стационарном обследовании (компьютерная мультиспиральная томография и аортоартериография) было установлено, что образование имеет торакоабдоминальное расположение с крупным питающим артериальным сосудом от верхней части абдоминального отдела аорты. Оперативное лечение включало в себя проведение заднебоковой торакотомии по VII межреберью справа. В ходе операции выявлена плотная адгезия кистозного образования с нижней долей правого лёгкого. После мобилизации опухоли от лёгкого, лигирования её сосудистого пучка установлено, что она уходит в брюшную полость через пищеводное отверстие диафрагмы. Рассечены ножки диафрагмы и тупым путём проведена полная мобилизация кисты от пищевода и желудка. Опухоль эвакуирована из плевральной полости, с последующим восстановлением целостности повреждённых тканей. Время операции составило 155 мин. Оперативное лечение проведено без интраоперационных осложнений, таких как кровотечение, перфорация пищевода, желудка. Дренаж из правой плевральной полости удалён на вторые сутки, признаков гидроторакса по данным ультразвукового исследования нет. Гистологическое заключение: зрелая кистозная тератома. На контрольной мультиспиральной компьютерной томографии грудной и брюшной полостей признаков резидульной ткани опухоли не обнаружено. Актуальность данной публикации обусловлена крайней редкостью торакоабдоминального расположения зрелых тератом у детей, а также отсутствием единых стандартов их хирургического лечения. Описание успешного радикального удаления крупного новообразования из единого торакотомного доступа расширяет клинические представления о топографо-анатомических особенностях таких опухолей и демонстрирует возможность минимизации операционной травмы при сочетанной локализации процесса.</p></trans-abstract><trans-abstract xml:lang="zh"><p>畸胎瘤是由源自三个胚层（内胚层、中胚层和外胚层）的细胞成分组成的新生物。畸胎瘤最常见的定位是骶尾部区域和卵巢（5160%）。但也可能存在其他定位：睾丸、腹膜后间隙、胃、颈和口腔底。身体两个不同腔室的畸胎瘤极为罕见。一名15岁女孩胸腔内的占位性病变是在进行计划性胸部X线检查时偶然发现的诊断结果。后续住院检查（计算机多层螺旋断层扫描和主动脉动脉造影）确定该病变位于胸腹部，有一支粗大的滋养动脉来自腹主动脉上段。手术治疗包括经右第7肋间后外侧胸切开术。 术中发现囊肿样形成与右下肺叶致密粘连。将肿瘤从肺部分离并结扎其血管束后，确定其通过膈肌食管裂孔进入腹腔。离断膈肌脚并以钝性方式完全游离囊肿与食管和胃。肿瘤从胸膜腔摘除，随后修复受损组织的完整性。手术时间：155分钟。手术治疗无术中并发症，如出血、食管或胃穿孔。右侧胸膜腔引流管于第二天拔除，超声检查未见胸腔积液迹象。组织学结论：成熟囊性畸胎瘤。胸腹腔多层螺旋计算机断层扫描未发现残留肿瘤组织迹象。本报告的现实性源于儿童胸腹部定位的成熟畸胎瘤极为罕见，且缺乏其外科治疗的统一标准。描述经单一胸切开入路成功根治性切除大型新生物，扩展了关于此类肿瘤局部解剖特征的临床认识，并展示了在组合定位过程中最小化手术创伤的可能性。</p></trans-abstract><kwd-group xml:lang="en"><kwd>mature teratoma</kwd><kwd>thoracoabdominal location</kwd><kwd>rare localization of teratoma</kwd><kwd>children</kwd><kwd>case report</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>зрелая тератома</kwd><kwd>торакоабдоминальная локализация</kwd><kwd>редкая локализация тератомы</kwd><kwd>дети</kwd><kwd>клинический случай</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>成熟畸胎瘤</kwd><kwd>胸腹部定位</kwd><kwd>畸胎瘤罕见定位</kwd><kwd>儿童</kwd><kwd>临床病例</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>National Society of Pediatric Hematologists and Oncologists.
