In some cases, cardiovascular complications are reported, including myocardial dysfunction, myocardial ischemia, shock, arrhythmias, coronaritis and coronary artery aneurysms. Although the incidence of coronavirus infection has decreased, patient mortality from multisystem inflammatory syndrome remains high. A clinical case of COVID-associated multisystem inflammatory syndrome in an infant is presented. Since admission, the patient with a high titer of immunoglobulin G with SARS-CoV-2 showed signs of hyperinflammation with subsequent development of pericarditis and myocardial dysfunction associated with prolonged bilateral pneumonia. Coronaritis and aneurysm of the left coronary artery were diagnosed. The patient had a long history of reduced global contractile function and high levels of myocardial damage markers requiring prolonged inotropic support and use of cardiac glycosides. Later, paroxysmal tachycardia developed in addition to cardiovascular complications. Comprehensive management of the disease and early detection of complications resulted in a favorable outcome.</p></abstract><trans-abstract xml:lang="ru"><p>Крайне тяжелая форма коронавирусной инфекции у детей ― мультисистемный воспалительный синдром, который характеризуется системным гипервоспалением с развитием синдрома полиорганной недостаточности. В ряде случаев регистрируются кардиоваскулярные осложнения в виде миокардиальной дисфункции, ишемии миокарда, шока, аритмии, развития коронарита и аневризм коронарных артерий. Несмотря на то что заболеваемость коронавирусной инфекцией снизилась, мультисистемный воспалительный синдром остается причиной высокой летальности пациентов. Представлен клинический случай мультисистемного воспалительного синдрома, ассоциированного с коронавирусной инфекцией, у грудного ребенка. С момента поступления на фоне длительно текущей двусторонней пневмонии у пациента с высоким титром иммуноглобулинов G с SARS-CoV-2 были выявлены признаки гипервоспаления с последующим развитием перикардита и миокардиальной дисфункции. Был диагностирован коронарит и аневризма левой коронарной артерии. Длительное время у пациента сохранялись сниженная глобальная сократительная функция и высокий уровень маркеров повреждения миокарда, что требовало продолжительной инотропной поддержки и назначения сердечных гликозидов. В последующем к кардиоваскулярным осложнениям присоединилась пароксизмальная тахикардия.
