However, cases of diagnosis in later age, including in adults, have been described. The clinical presentation is highly variable, and a significant amount of time often passes before the accurate diagnosis is made. This study presents a clinical case of a 17-year-old girl with chronic duodenal obstruction caused by isolated malrotation and fixation of the duodenum. Considering the patient’s comorbidities and developmental disorders, the diagnostic search was significantly challenging. In this situation, surgery involves mobilization of the duodenum and correction of the pathological fixation. However, considering the significant changes in the duodenal wall and the combined pathology, duodenojejunostomy was performed. Despite the additional surgical intervention, the postoperative period was characterized by prolonged gastric–duodenal stasis. Adequate diet, frequent feeding with a mixture, maximal “verticalization,” and activation of the patient played a key role in alleviating anastomositis and motor disturbances after the operation. Thus, using a strictly individualized approach to treat adolescents with a complex of surgical and therapeutic pathology and psychiatric disorders results in a satisfactory treatment outcome.</p></abstract><trans-abstract xml:lang="ru"><p>Осложнения мальротации кишечника чаще всего манифестируют в первые месяцы жизни, однако описаны случаи диагностики и в более позднем возрасте, в том числе у взрослых. Клиническая картина исключительно вариабельна, и до правильного диагноза нередко проходит немало времени. Представлено клиническое наблюдение хронической дуоденальной непроходимости у 17-летней девочки, вызванной изолированной мальротацией и гиперфиксацией двенадцатиперстной кишки. Учитывая коморбидность и нарушения психического развития пациентки, диагностический поиск был существенно затруднен. Оперативное вмешательство в данной ситуации предполагает мобилизацию дуоденум и устранение патологической фиксации. Однако, принимая во внимание выраженные изменения стенки двенадцатиперстной кишки, сочетанную патологию, было принято решение о выполнении дуоденоеюностомии. Несмотря на дополнительный оперативный прием, послеоперационный период характеризовался длительным сохранением гастродуоденостаза. Адекватная диета, дробное питание смесью, максимальная «вертикализация» и активизация пациентки сыграли ключевую роль в купировании явлений анастомозита и нарушений моторики после операции. Таким образом, строго индивидуальный подход в лечении подростка с комплексом хирургической, терапевтической патологии и нарушений психики позволил достичь удовлетворительного результата лечения.</p></trans-abstract><trans-abstract xml:lang="zh"><p>肠旋转不良的并发症最常出现在生命的头几个月，但也有在晚年诊断的病例，包括在成人中。临床情况变化很大，通常需要很长时间才能做出正确的诊断。报告一例17岁女孩因孤立性十二指肠旋转不良及过度固定所致慢性十二指肠梗阻的临床观察。考虑到患者的合并症和精神发育障碍，诊断搜索非常困难。这种情况下的手术干预涉及十二指肠的动员和病理性固定的消除。然而，考虑到十二指肠壁的明显变化，结合病理，决定进行十二指肠空肠吻合术。尽管进行了额外的手术入院，但术后的特点是胃十二指肠淤滞长期存在。充足的饮食、混合营养、最大限度的“垂直化”和患者的激活对于阻止术后吻合口炎和运动障碍的发生起着关键作用。因此，在治疗一名患有手术、治疗病理和精神障碍复合体的青少年时，严格个体化的方法取得了令人满意的治疗结果。</p></trans-abstract><kwd-group xml:lang="en"><kwd>megaduodenum</kwd><kwd>intestinal malrotation</kwd><kwd>chronic duodenal obstruction</kwd><kwd>case report</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>мегадуоденум</kwd><kwd>мальротация кишечника</kwd><kwd>хроническая дуоденальная непроходимость</kwd><kwd>клинический случай</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>巨十二指肠</kwd><kwd>肠旋转不良</kwd><kwd>慢性十二指肠梗阻</kwd><kwd>临床病例</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Sadler TW.
