Lung sequestration accounts for 0.15%–6.4% of all lung malformations. This study presents a clinical case of a 7-month-old girl with extralobaric lung sequestration. The patient presented with complaints of cough; noisy, rapid breathing; and a history of gastroesophageal reflux. Gastroenterological pathology was excluded at the place of residence. During physical examination, wet wheezing was heard on both lungs, and the respiratory rate was 36 per minute. Chest X-ray showed a right-sided, upper-lobe pneumonia. Community-acquired right-sided, upper-lobe pneumonia, moderate form, was diagnosed. CT scan of the chest organs with contrast revealed a congenital malformation: tracheal bronchus and extralobar lung sequestration on the right. Indications for minimally invasive intervention were formulated. After surgical treatment, thoracoscopy showed an extrapulmonary sequester in the posterior hemithorax, with a feeding vessel from the thoracic aorta. Then, sequestrectomy was performed. In the postoperative period, positive dynamics was observed against the background of antibacterial, infusion, and symptomatic therapy. The patient was discharged in satisfactory condition. In children with long-term, recurrent lung infections, without positive dynamics against the background of conservative therapy and in the presence of respiratory disorders against the background of normal body temperature and absence of signs of inflammation in blood tests, congenital malformations of the respiratory tract should be excluded. Computed tomography with contrast enhancement and subsequent 3B reconstruction is the most appropriate method for diagnosing lung sequestration. Moreover, thoracoscopic resection of a separate nonfunctioning fragment of lung tissue is an effective minimally invasive surgical treatment method.</p></abstract><trans-abstract xml:lang="ru"><p>Согласно данным литературы, частота диагностики пороков развития легких значительно возросла за последнее десятилетие и составляет 1 : 2500–3000 новорожденных. Секвестрация легкого представляет собой аномалию развития в виде отдельного нефункционирующего фрагмента ткани легкого, не сообщающегося с бронхиальным деревом и кровоснабжаемого аортой или артерий большого круга кровообращения. Секвестрация легкого составляет 0,15–6,4 % всех пороков развития легких. В статье представлен клинический случай экстралобарной секвестрации легкого у девочки 7 мес. Ребенок поступил с жалобами на кашель, шумное, учащенное дыхание, в анамнезе — клиника гастроэзофагального рефлюкса, гастроэнтерологическая патология исключена по месту жительства. При ретроспективном анализе у данного пациента выяснено, что пренатально порок развития не выявлен. При физикальном обследовании выслушивались влажные хрипы с обеих сторон, частота дыхательных движений составляла 36 в минуту, на рентгенограмме грудной клетки картина правосторонней, верхнедолевой пневмонии. Выставлен диагноз: «Внебольничная правосторонняя, верхнедолевая пневмония, среднетяжелая форма. Стадия ДН<sub>0-1</sub>». При проведении компьютерной томографии органов грудной клетки с контрастированием обнаружен врожденный порок развития — трахеальный бронх справа, экстралобарная секвестрации легкого справа. Сформулированы показания к миниинвазивному вмешательству. В ходе оперативного лечения при торакоскопии в заднем отделе гемиторакса выявлен внелегочный секвестр с питающим сосудом, исходящим от грудного отдела аорты, выполнена секвестрэктомия. В послеоперационном периоде на фоне антибактериальной, инфузионной, симптоматической терапии наблюдалась положительная динамика, ребенок выписан в удовлетворительном состоянии. У детей с длительно протекающими, рецидивирующими легочными инфекциями, без положительной динамики на фоне консервативной терапии и, особенно, при наличии респираторных нарушений на фоне нормальной температуры тела и отсутствии признаков воспаления в анализах крови необходимо исключать врожденные пороки развития дыхательных путей<italic>.</italic> Компьютерная томография с контрастным усилением и последующей 3D-реконструкцией наиболее информативный метод диагностики секвестрации легкого. В настоящее время торакоскопическая резекция отдельного нефункционирующего фрагмента легочной ткани является эффективным миниинвазивным хирургическим методом лечения.</p></trans-abstract><trans-abstract xml:lang="zh"><p>根据文献资料，肺畸形的诊断率在过去十年中显著增加，为1:25 00-3000新生儿。肺隔离是一种发育异常，表现为独立的无功能肺组织碎片，与支气管树不相连，由主动脉或大循环动脉供血。肺隔离症占所有肺畸形的0.15-6.4%。本文报告了一例7个月大女孩小叶外肺隔离症的临床病例。孩子入院时主诉咳嗽、嘈杂、呼吸急促，有胃食管反流诊所病史，居住地排除胃肠病学。通过对该患者的回顾性分析，明确了产前未发现畸形。体格检查时听到两侧湿啰音，呼吸频率为每分钟 36 次，胸部 X 光片显示为右侧上叶肺炎。诊断结果为：“院外右侧上叶肺炎，中重度。DN0-1期”。在对胸部器官进行对比CT扫描时，发现了先天性畸形——右侧气管支气管，右侧肺叶外隔离。制定了微创干预的适应症。 在胸腔镜手术治疗期间，在半胸后部发现肺外隔离，营养血管来自胸主动脉，并进行了隔离切除术。术后期间，在抗菌、输液、对症治疗的背景下，观察到积极的动态，患儿出院情况满意。对于患有长期、复发性肺部感染的儿童，在保守治疗的背景下没有积极的动态，特别是在体温正常且血液检查中没有炎症迹象的背景下存在呼吸系统疾病的情况下，有必要排除先天性呼吸道畸形。对比增强计算机断层扫描和随后的三维重建是诊断肺隔离症信息最丰富的方法。目前，胸腔镜下切除单个无功能肺组织碎片是一种有效的微创手术治疗方法。</p></trans-abstract><kwd-group xml:lang="en"><kwd>lung sequestration</kwd><kwd>extralobar sequestration</kwd><kwd>tracheal bronchus</kwd><kwd>case report</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>секвестрация легкого</kwd><kwd>экстралобарная секвестрация</kwd><kwd>трахеальный бронх</kwd><kwd>клинический случай</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>肺栓塞</kwd><kwd>肺外栓塞</kwd><kwd>气管支气管</kwd><kwd>临床病例</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Tumanova UN, Dorofeeva EI, Podurovskaya YuL, et al.
