This article highlights the successful treatment of a newborn child with renal vein thrombosis using systemic thrombolysis. A clinical case of successful systemic thrombolysis in a full-term boy in the early neonatal period with thrombosis of the left renal vein and transition of the floating part of the thrombus to the inferior vena cava is presented. A child aged 3 days was admitted to the neonatal intensive care unit from a maternity hospital, where thrombosis was suspected. Gross hematuria was observed. Laboratory data revealed thrombocytopenia and increased levels of fibrinogen and D-dimer. Ultrasound showed a sharp increase in the volume of the left kidney, a diffuse change in its parenchyma, a sharp depletion of blood flow, and accumulation of fluid in the perinephric tissue on the left, and a thrombus was observed in the lumen of the left renal vein with the presence of a floating part extending into the inferior vena cava. Thrombolytic therapy with the drug alteplase was started in combination with an infusion of unfractionated heparin. Doses were selected based on coagulogram parameters. On day 2 of therapy, hematuria resolved. On day 4 of therapy, the alteplase infusion was completed, and heparin therapy was continued. On day 13, the child was switched to a low-molecular-weight-heparin therapy. Dynamic ultrasound revealed size reduction, recanalization and further thrombus lysis, and normalization of intrarenal blood flow and kidney size. No renal dysfunction was recorded during observation. The child was transferred to the further care unit at aged 14 days and was discharged home at the age of 25 days in satisfactory condition on ongoing anticoagulant therapy under outpatient supervision of a hematologist. During follow-up observation at aged 1.5 months of life, no clinical, laboratory, and ultrasound indicators of renal function disorders were noted. Currently, there are no approved recommendations and protocols for individual thrombosis in newborns, although this particular age group is due to the vulnerability of hemostasis to the development of a thrombotic process.</p></abstract><trans-abstract xml:lang="ru"><p>Тромбозы, возникающие в неонатальном периоде — нозология, имеющая большое количество осложнений, нередко приводящих к инвалидности и летальным исходам. Цель публикации данной статьи — поделиться собственным опытом успешного лечения новорожденного ребенка с тромбозом почечной вены путем проведения системного тромболизиса. Представлен клинический случай успешного системного тромболизиса у доношенного мальчика в раннем неонатальном периоде с тромбозом левой почечной вены и переходом флотирующей части тромба в нижнюю полую вену. Ребенок в возрасте 3 сут жизни поступил в отделение реанимации и интенсивной терапии для новорожденных из родильного дома, где был заподозрен тромбоз. Обращала на себя внимание макрогематурия, в лабораторных данных определялась тромбоцитопения, повышение уровня фибриногена и D-димера, по данным ультразвукового исследования отмечалось резкое увеличение объема левой почки, диффузное изменение ее паренхимы, резкое обеднение кровотока, скопление жидкости в паранефральной клетчатке слева, также отчетливо визуализировался тромб в просвете левой почечной вены с наличием флотирующей части, распространяющейся в нижнюю полую вену. По решению консилиума была начата тромболитическая терапия препаратом алтеплаза в сочетании с инфузией нефракционированного гепарина, дозы подбирались под контролем показателей коагулограммы. На 2-е сутки терапии разрешилась гематурия. На 4-е сутки терапии завершилась инфузия алтеплазы, гепаринотерапия была продолжена. На 13-е сутки ребенок был переведен на терапию низкомолекулярным гепарином. При выполнении ультразвукового исследования в динамике выявлены признаки уменьшения размеров, реканализации и дальнейшего лизиса тромба, нормализации интраренального кровотока и размеров почки. Почечной дисфункции за время наблюдения зарегистрировано не было. Ребенка перевели в отделение дальнейшего выхаживания в возрасте 14 сут жизни и далее выписали домой в возрасте 25 сут жизни в удовлетворительном состоянии на продолжающейся антикоагулянтной терапии под амбулаторное наблюдение врача-гематолога. При катамнестическом наблюдении в возрасте 1,5 мес. жизни клинико-лабораторных и ультразвуковых показателей нарушений почечной функции не было. В настоящее время отсутствуют утвержденные рекомендации и протоколы по лечению тромбозов у новорожденных детей, хотя именно эта возрастная группа ввиду особенностей гемостаза уязвима по развитию тромботических состояний.</p></trans-abstract><trans-abstract xml:lang="zh"><p>新生儿期发生的血栓形成是一种具有大量并发症的疾病，往往会导致残疾和死亡。目的。发表这篇文章的目的，是分享我通过全身溶栓疗法成功治疗一名患有肾静脉血栓的新生儿的亲身经历。我们介绍了一例成功全身溶栓的临床病例，患者是一名早产男婴，新生儿早期左肾静脉血栓形成，血栓絮状部分进入下腔静脉。一名出生 3 天的患儿从妇产医院被送入新生儿重症监护室，当时怀疑患儿患上了血栓症。超声波检查显示左肾体积急剧增大，肾实质弥漫性改变，血流急剧减少，左侧肾旁细胞积液，左肾静脉管腔内也清晰可见血栓，血栓有一个絮状部分蔓延到下腔静脉。根据会诊的决定，开始使用阿替普酶联合输注非分叶肝素进行溶栓治疗；剂量是在控制凝血图参数的情况下选择的。血尿在治疗的第二天得到缓解。治疗第 4 天，阿替普酶输注结束，肝素治疗继续进行。第 13 天，患儿转为使用低分子量肝素治疗。超声波检查显示，患儿肾脏体积缩小，血栓再通并进一步溶解，肾内血流量和肾脏体积恢复正常。随访期间未发现肾功能异常。患儿在出生后 14 天转入护理部，在出生后 25 天出院回家，在血液科医生的门诊指导下继续接受抗凝治疗，情况令人满意。在出生后 1.5 个月的随访期间，临床、实验室和超声波检查均未发现肾功能异常指标。目前，虽然新生儿由于止血功能的特殊性很容易出现血栓形成的情况，但在治疗新生儿血栓形成方面还没有获得批准的建议和方案。</p></trans-abstract><kwd-group xml:lang="en"><kwd>neonatal thrombosis</kwd><kwd>newborns</kwd><kwd>thrombolytic therapy</kwd><kwd>venous thrombosis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>неонатальный тромбоз</kwd><kwd>новорожденный</kwd><kwd>тромболизис</kwd><kwd>венозный тромбоз</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>新生儿血栓</kwd><kwd>新生儿</kwd><kwd>溶栓</kwd><kwd>静脉血栓</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Kenet G, Cohen O, Bajorat T, Nowak-Göttl U.
