Diagnosis and choice of treatment tactics for this pathology often cause difficulties. A case report of a choledochal cyst in a 2-year-old patient with symptoms of recurrent jaundice, cholangitis, and pancreatitis, which required laparoscopic cholecystostomy and cholangiography during preparation for radical surgery, was retrospectively analyzed. The child was re-admitted to the pediatric surgical department with complaints of hyperthermia up to 39°C, resumption of periodic abdominal pain, nausea, and increased icterus of the sclera and skin. Laboratory and ultrasound examination indicated biliary and pancreatic hypertension. Choledochal cyst, cholestasis syndrome, acute cholecystopancreatitis was diagnosed. Owing to the existing clinical symptoms and laboratory and ultrasound data, laparoscopic cholecystostomy was urgently performed. Against the background of conservative therapy, within 2 days after laparoscopic cholecystostomy, clinically, laboratory and according to control ultrasound examination, biliary and pancreatic hypertension were stopped. The child underwent cholangiography through a cholecystostomy without additional anesthesia, which revealed a fusiform dilatation of the common hepatic and bile ducts. After 2 weeks of drug treatment against the background of laparoscopic cholecystostomy, the child underwent excision of the common bile duct cyst, and cholecystectomy with hepatic enteroanastomosis on a Roux-en-Y loop. The postoperative period was smooth. No other complaints or complications were noted within 24 months. In the reported case, recurrent obstructive jaundice was caused by a choledochal cyst. Laparoscopic cholecystostomy enabled the quick and successful relief of biliary hypertension, preparation of the patient for radical surgical treatment, and safe informative cholangiography through cholecystostomy. Hepatic enteroanastomosis with a disconnected loop according to Roux-en-Y was an effective surgical treatment method for choledochal cyst.</p> <p> </p></abstract><trans-abstract xml:lang="ru"><p>Кисты общего желчного протока или кисты холедоха представляют собой врожденную кистозную дилатацию внепеченочных и/или внутрипеченочных желчных протоков. Диагностика и выбор лечебной тактики при этой патологии нередко вызывают затруднения. Проведен ретроспективный анализ клинического случая кисты холедоха у пациентки 2 лет с симптомами рецидивирующей желтухи, холангита и панкреатита, потребовавших выполнения лапароскопической холецистостомии и холангиографии в период подготовки к радикальной операции. Ребенок повторно поступил в детское хирургическое отделение с жалобами на гипертермию до 39 °С, возобновление периодических болей в животе, тошноту, нарастание иктеричности склер и кожных покровов. Лабораторно и при ультразвуковом исследовании выявлены признаки желчной и панкреатической гипертензии. Установлен диагноз: «Киста холедоха. Синдром холестаза. Острый холецистопанкреатит». В связи с имеющимися клиническими симптомами, лабораторными и ультразвуковыми данными в срочном порядке проведена лапароскопическая холецистостомия. На фоне консервативной терапии, в течение 2 дней после лапароскопической холецистостомии клинически, лабораторно и по данным контрольного ультразвукового исследования желчная и панкреатическая гипертензия купированы. Ребенку выполнена холангиография через наложенную холецистостому без дополнительного наркоза, при которой выявлено веретенообразное расширение общего печеночного протока и общего желчного протока. Спустя 2 нед. медикаментозного лечения на фоне лапароскопической холецистостомии ребенку осуществлено иссечение кисты холедоха, а также холецистоэктомия с наложением гепатикоэнтероанастомоза на петле по Ру. Послеоперационный период протекал гладко. Других жалоб и осложнений в течение 24 мес. отмечено не было. В нашем наблюдении рецидивирующая механическая желтуха была обусловлена кистой холедоха. Лапароскопическая холецистостомия позволила быстро и успешно купировать желчную гипертензию и подготовить пациента к радикальному оперативному лечению, а также безопасно выполнить информативную холангиографию через холецистостому. Гепатикоэнтероанастомоз на отключенной петле по Ру оказался эффективным методом оперативного лечения кисты общего желчного протока.</p></trans-abstract><trans-abstract xml:lang="zh"><p>胆总管囊肿或胆总管囊肿是肝外和/或肝内胆管的先天性囊性扩张。这种病症的诊断和治疗策略的选择往往会造成困难。回顾性分析了一例胆总管囊肿的临床病例，患者两岁，有复发性黄疸、胆管炎和胰腺炎症状，在准备根治术期间需要进行腹腔镜胆囊造口术和胆管造影术。患儿再次入住小儿外科，主诉体温高达39°C，周期性腹痛复发，恶心，巩膜和皮肤黄疸增加。实验室和超声波检查发现了胆道和胰腺高血压的迹象。诊断结果是：胆总管囊肿、胆汁淤积综合征、急性胆囊-胰腺炎。根据临床症状、实验室和超声波检查数据，紧急实施了腹腔镜胆囊造口术。在保守治疗的背景下，腹腔镜胆囊造口术后 2 天内，根据临床、实验室和对照超声检查的数据，胆道和胰腺高血压得到了临床、实验室控制。在没有额外麻醉的情况下，患儿通过胆囊造口术进行了胆管造影，结果显示肝总管和胆总管呈纺锤形扩张。在腹腔镜胆囊造口术的背景下，经过两周的药物治疗后，患儿接受了胆总管囊肿切除术和胆囊切除术，并在Roux环路上进行了肝肠吻合术。术后恢复顺利。在24个月的时间里，患者没有出现其他不适和并发症。根据我们的观察，复发性机械性黄疸是由胆总管囊肿引起的。 腹腔镜胆囊造口术能够快速、成功地控制胆道高压，为病人接受根治性手术治疗做好准备，并通过胆囊造口术安全地进行信息胆管造影。事实证明，肝肠吻合术已被证明是外科治疗胆总管囊肿的有效方法。</p></trans-abstract><kwd-group xml:lang="en"><kwd>choledochal cyst</kwd><kwd>cholecystostomy</kwd><kwd>hepaticojejunostomy</kwd><kwd>children</kwd><kwd>case report</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>киста холедоха</kwd><kwd>холецистостомия</kwd><kwd>гепатикоэнтероанастомоз</kwd><kwd>дети</kwd><kwd>клинический случай</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>胆总管囊肿</kwd><kwd>胆囊造口术</kwd><kwd>肝肠吻合术</kwd><kwd>儿童</kwd><kwd>临床病例</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Razumovsky AYu, Rachkov VE.
