Seredavin Samara Regional Hospital, Samara. The patients were divided into two groups. The main group included 237 patients who underwent preoperative preparation according to the proposed scheme (depending on age, peritonitis stage, and degree of endotoxemia) lasting at least 4–6 h. In addition, for intracorporeal detoxification, the antihypoxant meglumine sodium succinate was used. The control group comprised 102 children who received traditional therapy according to generally accepted approaches and national guidelines for pediatric surgery lasting 2–3 h. Changes in hemodynamic parameters, electrolytes, hematocrit, platelets, and blood gases were comprehensively examined.</p> <p><bold>RESULTS</bold><italic>: </italic>After preoperative preparation, a statistically significant improvement in hemodynamic parameters was observed in the main group compared with that in the control group; the indicators were close to normal values. The dynamics of hematological parameters indicated more pronounced hemodilution in patients of the main group of all ages, which proves a significant improvement in the rheological properties of blood. Electrolyte and blood gas indicators did not reach the indicators of healthy children in any groups; however, in the main group, they were closer to normal. Complications of varying severity in the main group occurred in 21 children (8.8% ± 1.4%; <italic>p</italic> &lt; 0.001), whereas in the control group, they were diagnosed in 47 patients (46.0% ± 4.6%). A statistically significant reduction in the length of stay of patients in the intensive care unit after surgery was noted in 80% of children in the main group.</p> <p><bold>CONCLUSIONS</bold><italic>:<bold> </bold></italic>The proposed scheme of preoperative preparation based on a differential approach using the antihypoxant meglumine sodium demonstrated greater effectiveness than traditional treatment. The composition, volume, and duration of preoperative preparation are dependent on the state of homeostasis and the age of the patient.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность</bold>. Распространенный гнойный перитонит — наиболее тяжелое осложнение абдоминальной хирургической патологии, сопровождаемое развитием серьезных метаболических нарушений, устранение которых входит в задачу предоперационной подготовки.</p> <p><bold>Цель</bold> — сравнение традиционной предоперационной подготовки и новой схемы, предусматривающей дифференциальный подход с учетом стадии перитонита, степени эндотоксикоза, возраста ребенка и введения сукцината.</p> <p><bold>Материалы и методы</bold>. Проведен анализ результатов лечения пациентов в возрасте 1–14 лет (средний возраст 7,9 ± 3,9 года) с распространенным гнойным перитонитом различной этиологии, находившихся на лечении с 2006 по 2022 г. в детском хирургическом отделении Самарской областной больницы им. В.Д. Середавина, Самара. Пациенты были разделены на две группы. В основную вошли 237 человек, у которых проводилась предоперационная подготовка по предлагаемой схеме (в зависимости от возраста, стадии перитонита и степени эндотоксикоза) длительностью не менее 4–6 ч. С целью интракорпоральной дезинтоксикации в ней применяли антигипоксант меглюмина натрия сукцинат (Реамберин®). Контрольную группу составили 102 ребенка, которым проводилась традиционная терапия согласно общепринятым подходам и национальным руководствам по детской хирургии длительностью 2–3 ч. Комплексное обследование включало изучение изменений показателей гемодинамики, электролитов, гематокрита, тромбоцитов и газов крови.</p> <p><bold>Результаты</bold>. После предоперационной подготовки выявлено статистически более значимое улучшение показателей гемодинамики в основной группе по сравнению с контрольной, которые приближались к нормативным. Динамика гематологических показателей свидетельствовала о более выраженной гемодилюции у пациентов основной группы всех возрастов, что доказывает значительное улучшение реологических свойств крови. Показатели электролитов, газов крови не достигли показателей здоровых детей ни в одной из групп, но в основной они были более приближенными к нормальным. Осложнения различной степени тяжести в основной группе возникли у 21 ребенка (8,8 ± 1,4; <italic>p</italic> &lt; 0,001), тогда как в контрольной были диагностированы у 47 пациентов (46,0 ± 4,6 %). Статистически значимое уменьшение длительности пребывания пациентов в отделении реанимации и интенсивной терапии после операции отмечено у 80 % детей основной группы.</p> <p><bold>Выводы</bold>. 1. Предлагаемая схема предоперационной подготовки на основе дифференциального подхода с применением препарата Реамберин® продемонстрировала большую эффективность по сравнению с традиционным лечением. 2. Состав, объем и продолжительность проведения предоперационной подготовки зависят от состояния гомеостаза и возраста и пациента.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>论证</bold>。弥漫性化脓腹膜炎是腹部外科病理中最严重的并发症，并伴有严重的代谢紊乱。消除这些紊乱是术前准备的任务。</p> <p><bold>目的</bold>。本研究的目的是比较传统的术前准备和新的治疗方案，新治疗方案会考虑腹膜炎的阶段、 内毒素血症的程度、患儿的年龄和琥珀酸盐的用量。</p> <p><bold>材料与方法</bold>。对2006年至2022年在V.D.
