One of the basic factors that influence treatment outcomes in the future is a thorough study of infusion therapy methods in newborns, considering the anatomical and physiological characteristics of the neonatal period.</p> <p><bold>AIM</bold><italic>:</italic> This study aimed to examine the electrolyte balance, acid–base state, and hemodynamic parameters in newborns, depending on the choice of basic infusion medium during surgery.</p> <p><bold>MATERIALS AND METHODS</bold><italic>:</italic> A prospective study was conducted in 99 newborns. All children were given an infusion of saline solutions of 10 mL/kg/h during surgery. Groups I, II, and III were administered intraoperatively with isotonic Sterofundin, saline solution, and Ringer’s solution, respectively. A hypotonic sodium chloride solution of 0.45% was administered to 11 children. The indicators of the acid–base state and electrolyte composition of the venous blood, hemodynamic parameters, and need for perioperative inotropic support were evaluated.</p> <p><bold>RESULTS</bold><italic>:</italic> A tendency to hyponatremia was detected in children who were infused with 0.45% sodium chloride hypotonic solution, which resulted in the abandonment of further recruitment of children in the group and the implementation of this study option. After surgery, the pH-compensated state was maintained in all three groups. Moreover, metabolic disorders in the form of a decrease in bicarbonates and ВE were recorded in all groups. Electrolyte disturbances were detected in all groups, whereas electrolyte balance was most often registered in group I. In groups II and III, common ionogram findings included a decrease in potassium and an increase in sodium, chlorine, and calcium levels. In the analysis of the hemodynamic parameters, no significant statistical difference was recorded during surgery when comparing the groups infused with different saline media.</p> <p><bold>CONCLUSIONS</bold><italic>: </italic>In this study, no significant differences were found in the indicators of acid–base state and hemodynamic parameters when using different saline solutions as basic intraoperative infusion therapy in newborns. Regarding the electrolyte balance, the most common complications were hypokalemia, hypernatremia, and hyperchloremia in the 0% saline group.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность</bold>. В последние годы неонатальная хирургия развивается и совершенствуется, в том числе благодаря более глубокому изучению анестезиологических подходов и внедрению полученных знаний в повседневную практику врача – анестезиолога-реаниматолога. Одним из базовых факторов, оказывающим влияние на исход лечения в дальнейшем, является тщательное изучение методов инфузионной терапии у новорожденных детей с учетом анатомо-физиологических особенностей неонатального периода.</p> <p><bold>Цель</bold><italic> — </italic>изучение электролитного баланса, показателей кислотно-основного состояния и гемодинамических показателей у новорожденных в зависимости от выбора базовой инфузионной среды во время хирургической операции.</p> <p><bold>Материалы и методы</bold>. Проведено проспективное исследование у 99 детей периода новорожденности. Все дети во время оперативного вмешательства получали инфузию солевых растворов в объеме 10 мл/(кг · ч). В группе 1 интраоперационно вводили Стерофундин изотонический, в группе 2 — 0,9 % раствор натрия хлорида, в группе 3 — раствор Рингера. У 11 детей вводился 0,45 % гипотонический раствор натрия хлорида. Оценивали показатели кислотно-основного состояния и электролитного состава венозной крови, гемодинамические показатели и потребность в периоперационной инотропной поддержке.</p> <p><bold>Результаты</bold>. У детей, которым вводили 0,45 % гипотонический раствор натрия хлорида зафиксировано появление тенденции к гипонатриемии, что послужило отказом от дальнейшего набора детей в группу и проведения данного варианта исследования. После оперативного вмешательства в трех группах сохранялось компенсированное по рН состояние. При этом во всех группах зафиксированы метаболические нарушения в виде снижения бикарбонатов и ВЕ. Во всех группах были выявлены электролитные нарушения, при этом в группе 1 наиболее часто зарегистрирован электролитный баланс; в группах 2 и 3 более часто встречались нарушения ионограммы в виде снижения содержания калия, повышения содержания натрия, хлора и кальция. При исследовании гемодинамических показателей во время оперативного вмешательства не зафиксировано достоверной статистической разницы при сравнении их между группами, в которых проводилось вливание разных солевых сред.</p> <p><bold>Заключение</bold>. По итогам работы не выявлено значимых различий в показателях кислотно-основного состояния и гемодинамических показателях при использовании разных солевых растворов в качестве базовой интраоперационной инфузионной терапии у новорожденных. При изучении электролитного баланса наиболее частыми осложнениями стали гипокалиемия, гипернатриемия и гиперхлоремия в группе с применением 0,9 % раствора натрия хлорида.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>论证</bold>。近年来，新生儿外科手术不断发展和进步，其中包括对麻醉方法的深入研究，以及将所学知识引入医生-麻醉师-复苏师的日常工作中。考虑到新生儿期的解剖生理特点，对新生儿输液治疗方法进行深入研究是影响未来治疗效果的基本因素之一。</p> <p><bold>目的</bold>。本文旨在研究新生儿在手术过程中与基础输液环境选择有关的电解质平衡、酸碱指标和血流动力学指标。</p> <p><bold>材料和方法</bold>。我们对99名新生儿进行了前瞻性研究。在手术过程中，所有患儿都接受了生理盐水输注，输注量为10毫升/（千克-小时）。I组术中注射等渗的 苹果酸电解质静注液，II组注射0.9%氯化钠溶液，III组注射林格氏溶液。为11名儿童注射了0.45%低渗氯化钠溶液。对静脉血的酸碱状态和电解质成分、血流动力学参数以及围手术期肌力支持的需求进行了评估。</p> <p><bold>结果</bold>。在接受0.45%低渗氯化钠溶液治疗的患儿中出现了低钠血症的倾向，这也是拒绝进一步招募患儿加入该组和进行该变体研究的原因。手术干预后，三组患儿的pH值均保持代偿状态。同时，在所有组中，都记录到了碳酸氢盐和BE减少形式的代谢紊乱。所有组别都发现了电解质紊乱，其中电解质平衡在I组最为常见；离子图紊乱在II组和III组更为常见，表现为钾降低、钠升高、氯升高和钙升高。在对手术干预期间的血流动力学参数进行的研究中，在输注不同生理盐水介质的各组之间进行比较，没有发现明显的统计学差异。</p> <p><bold>结论</bold>。我们的研究结果表明，在新生儿术中使用不同的生理盐水作为基本输注疗法时，酸碱状态和血流动力学参数没有明显差异。在研究电解质平衡时，使用0.9%氯化钠溶液组最常见的并发症是低钾血症、高钠血症和高氯血症。</p></trans-abstract><kwd-group xml:lang="en"><kwd>pediatric surgery</kwd><kwd>surgical interventions</kwd><kwd>intensive care</kwd><kwd>intraoperative infusion</kwd><kwd>electrolyte composition</kwd><kwd>newborns</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>детская хирургия</kwd><kwd>оперативные вмешательства</kwd><kwd>интенсивная терапия</kwd><kwd>интраоперационная инфузия</kwd><kwd>электролитный состав</kwd><kwd>новорожденные</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>儿科手术</kwd><kwd>外科干预</kwd><kwd>强化治疗</kwd><kwd>术中输液</kwd><kwd>电解质成分</kwd><kwd>新生儿</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Visram AR.
