Abstract
This commentary addresses the article by Kozlov Yu.A., Rozhanski A.P., Sapukhin E.V., Strashinsky A.S., Marchuk A.A., Ryakhina A.O., Semshchikova Yu.P., Makarochkina M.V., titled “Robot-Assisted Resection of a Common Bile Duct Cyst With Hepaticoduodenostomy in a Child: A Case Report,” published in the Russian Journal of Pediatric Surgery, Anesthesia and Intensive Care. 2026. Vol. 16, No. 1. P. 127–136. DOI: https://doi.org/10.17816/psaic1956. Based on the author’s own experience with similar laparoscopic procedures for choledochal cysts using hepaticoduodenostomy, a critical opinion is expressed regarding the appropriateness of this technique in children. Follow-up examinations revealed fasting bile accumulation in the stomach. Due to the impossibility of long-term outcome prediction with this procedure, the surgical team at the author’s clinic reverted to a Roux-en-Y loop.

Операция холедоходуоденоанастомоза при кистах холедоха создана специально для лапароскопии и сейчас применяется в роботической хирургии. Это оказалось гораздо проще сделать технически, чем создавать анастомоз по Ру. Несколько лет назад, следуя примеру наших коллег из Азии, мы выполнили серию аналогичных лапароскопических операций у детей. Но когда проследили результаты этих вмешательств, то увидели у всех пациентов скопление большого количества желчи в желудке натощак. Этот феномен, как правило, у детей не имеет чётко выраженных жалоб, что позволяет хирургам не концентрировать на нём внимание. Поскольку оперативные вмешательства у детей направлены на благополучный исход не только в краткосрочной перспективе, а должны учитывать и долгосрочный прогноз (20 лет и более), мы прекратили эти операции и вернулись к петле по Ру. Это короткое замечание может заставить хирургов задуматься при внедрении описанной авторами статьи операции в практику.
